Skilled Nursing Care Services
Each P&N Vision Home Health Care client receives a personal, in-depth assessment conducted by our Director of Nursing that, among other things, takes into account their condition, personality and environment. We then put that knowledge to use, creating an individualized plan of care that meets our clients’ unique physical and emotional needs – a plan that strives to not only improve their health, but enhance their quality of life. This process, along with continual oversight by our nursing staff, helps us ensure the right fit between our clients and their caregivers, and enables us to quickly modify a client’s plan of care as their needs evolve.
Daily Living Assistance
Our clinically based model of care provides full client oversight, including clinical and environmental assessments, supervision of our RNs, LPNs and CNAs, and comprehensive case management to make sure you and your loved ones receive the highest quality of care.
P&N Vision Home Health Care also works hand-in-hand with physicians, referral sources and other healthcare professionals to ensure continuity and coordination of care, affecting the best possible outcomes for our clients.

Personal home care—or private duty care—makes it possible for older adults to age in place while living with physical disabilities or a chronic health condition. This type of home care includes help with personal activities, such as dressing, bathing, and mobility. Seniors who receive private or personal home care can maintain their independence and continue living at home, rather than moving to a care facility.
At P&N Vision Home Health Care, we know how important the comfort of home can be for seniors. If your loved one is worried about losing their independence or finds themselves struggling with everyday tasks, Visiting Angels can help your loved receive the care they need to age in place on their own terms.
What Is Personal Care?
Personal care is one of the most common types of in-home care. Generally, this type of care is meant for individuals with chronic health conditions, physical disabilities, or difficulties with strength, coordination, and mobility. Care will typically include a combination of tasks and activities, which are designed to ensure the individual's safety, comfort, and well-being.
One of the most important aspects of personal home care (or private care) is assistance with Activities of Daily Living, also known as ADLs. These activities include things like dressing, personal grooming, bathing, and toileting. Personal caregivers have the skills and expertise required to provide assistance with these tasks, as well as similar tasks such as mobility and transfer assistance.
Personal caregivers can offer a helping hand with a number of other tasks, including assistance with errands, meal preparation, light housekeeping, medication reminders, and other routine activities. Our caregivers also provide individuals with companionship and conversation, helping reduce the risks of senior isolation.
Personal Caregiving vs. Companion Caregiving
When hiring in-home care for a loved one, it can be hard to distinguish between different types of caregiving. In particular, many families are unsure of the distinction between personal care and companion care, which are two of the most common types of in-home care.
The core distinction between these two types of care is help with Activities of Daily Living, as well as any other caregiving tasks that direct physical assistance. Personal caregivers have specific skills and abilities that allow them to provide direct physical assistance, while also performing other caregiving duties.
Companion caregivers, on the other hand, can assist with activities such as errands or light housekeeping. However, they are unable to assist with Activities of Daily Living or perform other tasks that involve physical assistance.
Because of this distinction, we recommend personal care services if your loved one requires direct physical assistance. We may also recommend this type of care if your loved one has a chronic health condition, such as diabetes or dementia. If your loved one does not require this level of assistance, we may recommend companion care services instead.

Companion care for seniors provides assistance older adults need to live safely and comfortably at home. This type of home care covers a range of basic tasks and activities that many seniors find difficult to perform on their own. In other situations, these obstacles can put seniors at risk of losing their independence. But with companion care, seniors can maintain their routines and continue to age in place.
At P&N Vision Home Health Care, companion caregiving is one of our most popular services, covering a broad range of caregiving duties. This makes it a great choice for seniors who need a bit of help to remain at home, but who don't require advanced caregiving.
What Is Companion Care?
Companion care is a type of in-home care service that provides essential assistance to the elderly and disabled adults.
Care will typically include a number of ways to make daily living easier and safer for the care recipient. For example, a caregiver might perform errands, such as grocery shopping or picking up medications, on behalf of a senior. At other times, caregivers will plan and prepare regular meals, perform light housekeeping duties, and offer medication reminders.
This type of elder care is also designed to improve quality of life in other ways. Caregivers act as partners for favorite hobbies and activities, accompany clients to appointments and social outings, and engage in friendly conversation with clients.
These activities can be a lifeline for socially isolated seniors. Social isolation has shown to dramatically increase the risk of health complications and mortality in seniors. Companion caregiving provides isolated seniors with social interaction and regular companionship, ending the cycle of senior isolation.
Companion Caregiving vs. Personal Caregiving
When choosing elder care services for your loved one, you may need to choose between companion care and personal care. Because of this, it's helpful to know the difference between the two services.
With a home care companion, duties typically include activities such as doing laundry, making meals, and assisting with errands. However, this type of care does not include physical assistance with activities like dressing, bathing, or toileting.
Personal care covers the basic care duties of companion caregiving, while also providing physical assistance to clients. Personal care services can also include non-medical assistance for chronic health conditions, like diabetes or Parkinson's.
Most older adults who receive senior care can age in place comfortably with companion caregiving. However, seniors who require advanced care or physical assistance may need personal care to live at home safely.
